Credit Card Authorization
Authorize secure credit card payments to release deliveries faster—ideal for urgent replenishment.

- Cardholder signature required.
ACH Payment Authorization
Enable ACH / bank draft to reduce admin time for recurring invoices and scheduled deliveries.
- Provide bank name, routing & account number.
- Billing contact email for receipts.
Payment Agreement (Terms & Guaranty)
Standard credit terms for accounts requesting net terms or scheduled payments.
- 18% annual interest on delinquent balances.
- Collection costs & jurisdiction (Orange County, FL).
Dispenser Lease Form
Enroll in the dispenser program to improve hand hygiene and cost control with approved refills.
- Ownership of dispensers remains with USA Hospitality.

Account FAQ
What is the difference between a regular account and a business account?
Regular account is for online shopping and reorders. Business account is for wholesale terms, contract pricing, and multi-location support.
How long does it take to review a business account?
Most business accounts are reviewed within 1–2 business days once we receive your completed application and required documents. Timing may vary based on completeness.
Do I need to be tax-exempt to open an account?
No. Tax-exempt status is optional. If applicable, attach your DR-13 (Florida) or state exemption certificate.
Can we set up multiple locations under one business account?
Yes. We can set up multiple ship-to locations under one master account to control pricing, product lists, and invoices by property or department.
Can I sign the PDF forms electronically?
Yes. Print-sign-scan is fine, or use your preferred e-sign tool and email the signed PDFs to admin@usahospitality.com.
