Submit Everything Online

The fastest way to open an account is to complete our secure online registration form. Upload documents directly or email them after you submit.

  2. Fill in your company and billing information.
  3. Attach copies of ID and licenses or send them by email.

Email Signed Forms

Prefer PDF forms? Download the Customer Application and payment forms, sign them, and email everything to our credit department.

Business Account FAQ

How long does it take to open an account?

Most business accounts are reviewed within 1–2 business days once we receive your completed application and supporting documents. Time may vary based on credit review and completeness of information.

Do I need to be tax-exempt to open an account?

No. Tax-exempt status is optional. If your organization is tax-exempt, attach your DR-13 (Florida) or applicable state exemption certificate with your Customer Application.

Can we have multiple locations under one account?

Yes. We can set up multiple ship-to locations under a single master account so you can control pricing, products and invoices by property or department.

Can I sign these forms electronically?

Yes. Print-sign-scan is fine, or use your preferred e-signature tool and email the signed PDF to admin@usahospitality.com.
